IVF or IUI: how do you choose?
The key differences between the two most common fertility treatments, and how a specialist decides which one to start with.

IUI and IVF are the two fertility treatments people hear about most. They work very differently, and the right choice depends on your situation.
The basic difference
IUI helps sperm get closer to the egg. Prepared sperm is placed in the uterus around ovulation, and fertilisation still happens naturally inside the body.
IVF takes fertilisation into the lab. Eggs are collected, fertilised outside the body, and an embryo is transferred back into the uterus.
At a glance
| IUI | IVF | |
|---|---|---|
| How invasive | Minimal, no anaesthesia | Egg collection under light anaesthesia |
| Medicines | None, or mild ovulation support | Daily injections for about 10–12 days |
| Tubes need to be open? | Yes | No |
| Success per cycle | Lower | Higher |
| Cost | Lower | Higher |
When IUI may come first
- Younger couples with open tubes
- Mild sperm problems
- Ovulation problems such as PCOS
- Unexplained infertility
When going straight to IVF makes sense
- Blocked or damaged tubes
- Significant sperm problems
- Low egg reserve or older age
- Several IUI cycles that haven’t worked
The right answer is personal
Neither treatment is “better” for everyone. A good fertility evaluation shows which one gives you the best chance with the least strain on you, emotionally, physically and financially.
